Share Your Experience of Palliative and End of Life Care

NHS Cheshire and Merseyside have established a Palliative and End of Life Care Programme Board to ensure that everyone across our region can access high-quality, equitable end of life care.

They are seeking feedback to better understand people’s experiences of palliative and end of life care. Your insights will help them improve the quality, compassion, and consistency of care for individuals and families in the future.
This survey is for anyone who has experienced end of life care personally, or supported a loved one through it, within the past two years. 

There are three versions of the survey, each tailored to:

  • People currently receiving palliative or end of life care
  • Relatives and friends of someone currently receiving this care
  • People who have been bereaved in the last two years

The surveys will be open from November 2025 until the end of January 2026.
